Good catch web formproject

This form consists of fields that are sent via a POST request to an Azure Logic App. This logic app then moves the data to Dynamics 365 for storage and review by the Health and Safety team.

This form contains validation for the user and 'Sweet Alert' popup on successful submission. Production application being used globally across sites at Synergy, increasing the efficency and recording of good catches for the Health and Safety team.
