Good Catch web formproject

This form consists of fields that are sent via a POST request to an Azure Logic App. This logic app then moves the data to a model driven app created in the Power Platform for storage and review by the Health and Safety team.

This form contains validation for the user and the ability to upload images from the camera roll and camera, being stored in SharePoint on successful submission. The application is used globally across sites at Synergy, increasing the efficiency and recording of Good Catches for the Health and Safety team.
